What is Home Care Service?
Home Care Service is provided to
- support rehabilitation or convalescence
- facilitate hospital or nursing home discharge
- prevent or reduce inappropriate institutionalization
- strengthen family life
- relieve family members
- or perform other activities which enable comfortable daily living
Trained Home Care Aides who live in your community are assigned to each case according to their ability to adapt to individual needs of each family and to carry out a plan of care as designated by the Home Care Director, Case Manager, RN or Social Worker.
The Agency is under the auspices of the Franklin County Board of Health and the Franklin County Board of Supervisors. It is a non-profit organization with reimbursements made from Franklin County Public Health, private insurance, Veterans Affairs, Iowa Department of Public Health, Elderbridge Agency on Aging, waivers, client fees, donations, and local tax money.
